An affidavit is a legal document used in court and other law-authorized purposes, made by one individual in the presence of an authorized affidavit taker. Check specific court or tribunal websites for their affidavit format requirements. You can complete most of the affidavit form before visiting the authorized taker, but do not sign it until you are in their presence. A list of authorized affidavit takers is available at justice.vic.gov.au. During the meeting, you must sign or initial changes, sign each page, and the affidavit taker must also sign or initial alterations and sign every page. If the affidavit references a document, the affidavit taker must attach a certificate identifying that document.
