An affidavit must be made by one person in the presence of an authorized affidavit taker. Different courts and tribunals may have specific format requirements, so it's advisable to check their websites prior to completion. Most of the affidavit form can be filled out before meeting the affidavit taker, but it must be signed in their presence. A list of authorized affidavit takers is available at justice.vic.gov.au. The affidavit must be signed or initialed on any changes, with the taker also signing and initialing, and if the affidavit references a document, the taker must attach a certificate identifying it.
