Hi everyone, Kevin here. Today I want to show you how you can create your very own fillable form using Microsoft Word. First off, what is a fillable form? You can have people go in and fill out a form, but they wont be able to edit the questions or the structure of the form. You can use things like check boxes, text fields, drop down lists, date pickers, to build your form. Once someones finished completing your form, they can e-mail it back, they could print it out, or you could even connect it to a database. Connecting to a database is outside of the scope of todays tutorial. Its kind of like an docHub PDF form, but its Microsofts version of it. Ive included sample files today if you want to follow along, otherwise lets jump on the PC and lets get started. You can find it under File Options Customize Ribbon. Check the Developer box in the right-hand column. A list of all the medications you use, the dosages and how long youve been taking them. The dates of your doctors visits.
Include information on major medical conditions, causes of death, age at disease diagnosis, age at death, and ethnic background. Be sure to update the information regularly and share what youve learned with your family and with your doctor.
