In this tutorial, Kevin explains how to create a fillable form using Microsoft Word. A fillable form allows users to enter information without modifying the questions or structure. Features include checkboxes, text fields, dropdown lists, and date pickers. Once completed, users can email the form, print it, or connect it to a database, though database connections are not covered in this session. He mentions that the tutorial is similar to creating forms in DocHub but focuses on Microsoft Word. Sample files are provided for those who want to follow along, and he starts by working with an order form for the Kevin Cookie Company.

Did you know that there are two different types of DNR orders that can be chosen? The first is the DNR Comfort Care (DNRCC) and the other is the DNR Comfort Care- Arrest (DNRCC-Arrest).
DNR means that no CPR (chest compressions, cardiac drugs, or placement of a breathing tube) will be performed. A DNI or Do Not Intubate order means that chest compressions and cardiac drugs may be used, but no breathing tube will be placed.
Do not resuscitate orders. You have the option to complete a do not resuscitate order. This signed document instructs medical professionals not to perform CPR (cardiopulmonary resuscitation) if your heart or breathing stops.
A Data Release Number (DRN) is a four-digit number assigned to your Free Application for Federal Student Aid (FAFSA) form by the U.S. Department of Education. It is printed on the upper right corner of the paper Student Aid Report (SAR), in the upper right corner on the electronic SAR, and on your confirmation page.
How do I get a DNR form in Ontario? The DNR / Validity Order Form can be ordered through the MOHLTC Forms Warehouse by quoting Form Number: 3698-45 or Catalogue Number: 7350-5558.
You decide deciding in advance to refuse CPR You can make it clear to your doctor or medical team that you do not want CPR if your heart or breathing stops. Your doctor may complete a DNACPR form to indicate this and put it in your medical records.
Generally, a DNR order should not be entered into the patients medical record without concurrence of the parents. However, a DNR order may be entered by the patients physician without parental permission if CPR is determined to be physiologically futile or non-therapeutic, in ance with Paragraphs E and F below.
A DNR order does not mean do not treat. Rather, it means only that CPR will not be attempted. Other treatments (for example, antibiotic therapy, transfusions, dialysis, or use of a ventilator) that may prolong life can still be provided.
