In this video tutorial, the process of graph preparation for an inside ACL reconstruction using a quadrupled semitendinosus tendon is demonstrated. The 27-centimeter graph can be quadrupled to give a length between 65 and 70 millimeters. The ACL tightropes are loaded onto the graph preparation station to allow for solo preparation. The semitendinosus graft is passed through the ACL tightropes to create a quadruple graft with two free ends, which are sutured together using zero fiber wire. Incorporating this knot into the graph link increases the pullout strength.
