Answer. The format of an authorization letter should include the date, the name of the person to whom it is addressed, details about the person who has been authorized (such as name and identity proof), the reason for his absence, the duration of the authorized letter, and the action to be performed by another person.

The authorization letter format includes the address and date, salutation, body of the letter with the name and signature of the person you are authorizing, the reason for unavailability, complimentary closing, signature and name of the authorizer.
Authorization letter is usually written by somebody who cannot do or perform certain actions or business. Due to his absence or his physical or mental challenge and many other reasons, this person usually asks somebody else to do his/her business on his/her behalf.
