one common misconception is if somebody is a do not resuscitate then we stop treating them completely do not resuscitate means if somebodys heart stops beating on its own then we do not push on their chest we do not shock their heart we just let them die peacefully if somebody comes to the hospital and has say pneumonia and needs a course of antibiotics and they tell us that they are do not resuscitate that means that we give them the appropriate treatment up until the point that their heart stops if you do not want to be resuscitated or treated for a medical condition you have to be clear with your physician and your care team what exactly your wishes are

Important! In order to be legally valid this form MUST be printed on yellow paper prior to being completed. EMS and medical personnel are only required to honor the form if it is printed on yellow paper. Florida residents can also call the Florida Legislative Division Toll-free: 1-800-342-1827.
The DNR form also includes a wallet sized card that can be laminated and worn around the neck or inserted into a DNR bracelet that can be worn at all times.
A Do Not Resuscitate Order (DNRO) is a form developed by the Department of Health to identify people who do not wish to be resuscitated in the event of respiratory or cardiac arrest. A form may be obtained by downloading directly from this site. This form should only be printed on yellow paper (of any shade).
A DNR shirt cant serve as a DNR order, and so EMS will ignore it. With physicians, its more complicated -- they have to obey your wishes whether or not you filled out the state form, and so if you clearly express a wish not to be resuscitated they will issue a DNR order.
Documentation in the patients medical record of a decision not to do CPR must include both a Physicians Order and an entry in the Progress Notes. Use of the standardized DNR Order Form is preferred, but is not required in cases where the attending physician prefers a narrative method of documentation.
In 2008, the American Hospital Association (AHA) asked all U.S. hospitals to consider standardizing the color of their patient hospital wristbands as an improved safety measure, with these specific meanings: Purple to denote a patients Do Not Resuscitate (DNR) status. Red to denote a patients Allergy
In summary, the minimum requirement in order for a DNR form to be valid is the valid signature and date of the patient, or of their legally recognized healthcare agent in the event that they are incapacitated or otherwise unable to express their wishes.
Per CCCC, it is recommended that POLST be copied on bright pink paper to help ensure that the document stands out and is followed. However, POLST on any color paper is valid.
