we have very very busy days in the hospital were taking care of an inpatient service there are lots of responsibilities lots of duties people need to get home and its not trivial to expect providers to do discharge summaries at the end of a busy day after theyve discharged four or five patients but it should be expected when I came here three years ago it was acceptable for a discharge summary to be done 29 days after the person left the hospital 30 days was the cutoff is now down to 14 days in my opinion thats still unconscionable the reason a discharge summary is so critical its your link and your hand off to the communicating doctors if a provider outside the hospital doesnt know what happened in the hospital theyre at a disadvantage in terms of picking next steps that need to be undertaken immediately post discharge there may be important items to follow up on there may be important decisions that we made about medications that they need to understand when we queried physici

Thus, the Discharge Summary has pretty clear mandatory elements: what was the patients history, why were they hospitalized, what were the docHub events during their stay including procedures and treatments, in what condition did the patient leave the hospital, and what sort of follow-ups are required after
A discharge from hospital is the formal release of a patient from a hospital after a procedure or course of treatment. A discharge occurs whenever a patient leaves hospital upon completion of treatment, signing out against medical advice, transferring to another healthcare institution, or on death.
Its not wrong, but we would normally say, She was discharged from hospital. You are released from something that was incarcerating you, like prison: She was released from prison.
Most discharge letters include a section that summarises the key information of the patients hospital stay in patient-friendly language, including investigation results, diagnoses, management and follow up.
The steps to writing the letter are fairly straightforward as well: Get straight to the point. Provide the reason(s) for discharge. Give the patient a clear, dated deadline to find a new provider. Mention emergency care. Offer to transfer medical records.
A discharge summary will have been written by the doctor who was responsible for your care while you were in hospital. This is so your GP knows what tests and treatment youve had.
You should be able to get a copy from the ward manager or the hospitals Patient Advice and Liaison Service (PALS).
An unsafe discharge from hospital occurs if you are discharged, or sent home, when your release from the hospital could adversely affect your health. An unsafe discharge occurs in situations including if: You are sent home prior to being properly diagnosed. You are sent home when your condition has not yet stabilized.
