The DNR Form Ontario requires the patients name, date of birth, address, telephone number, and signature. Additionally, it requires the patient to specify their medical wishes regarding cardiopulmonary resuscitation (CPR), medical treatments, and other end-of-life decisions.
You can write an advance directive in several ways: Use a form provided by your doctor. Write your wishes down by yourself. Call your health department or state department on aging to get a form. Call a lawyer. Use a computer software package for legal documents.
For example, someone with terminal cancer might write that they do not want to be put on a respirator (breathing machine) if they stop breathing. This action can increase their control over their death and their peace of mind, and eventually reduce their suffering.
Documentation in the patients medical record of a decision not to do CPR must include both a Physicians Order and an entry in the Progress Notes. Use of the standardized DNR Order Form is preferred, but is not required in cases where the attending physician prefers a narrative method of documentation.
