Now were going to talk about Forms and Questionnaires. Go here to Evaluations and select Forms. Its super easy to create a form. Lets create a new one. Well call it Register bodyweight. Here on the right side are the tests or information fields that are in this form. Right now its empty. Down here you can add tests, information fields or training load. We want this to register bodyweight. We click Add Tests, this opens up the Test Collection. The Bodyweight test is here. I also want to include a question, so I click on Add Information, which opens up the Information Collection. I want to add a field where the athletes can write a little bit about how they feel. I dont have that item in my collection but I can create it on the fly right here. Ill use the Plain text option. Thats it, now I have a form ready to send it out.
