[Music] the DNR or do not resuscitate is perhaps the best known Advanced Directive the DNR is very specific a DNR says this and only this if you are found without a pulse and respiration medical staff wont try to restart your heart or breathing with either chest compressions shocks other machines or tubes or medications its important that you know an order for DNR is a choice it is not mandatory its a choice that you should make when you are in a condition to do so if you are in a hospital nursing home or assisted living a DNR is an agreement between you and the facility if your heart stops beating or you stop breathing it is your choice that nothing be done to resuscitate you if you do not have a DNR in place your agent that you named in your Healthcare Pro will make the decision about resuscitation for example if I was going to if I was hit with a car and you know on the way home and I was in an accident I would want my son who is my healthc care proxy to be able to talk with the

The DNR Form Ontario requires the patients name, date of birth, address, telephone number, and signature. Additionally, it requires the patient to specify their medical wishes regarding cardiopulmonary resuscitation (CPR), medical treatments, and other end-of-life decisions.
I understand DNR means that if my heart stops beating or if I stop breathing, no medical procedure to restart breathing or heart functioning will be instituted. I understand this decision will not prevent me from obtaining other emergency medical care by health care professionals prior to my death.
If you decide to have a DNR order, ask your doctor to explain and write it. Place the order in your home where everyone can easily see it. Consider naming a substitute decision-maker. This allows you to name a person to make decisions about your care if you are not able to.
Documentation in the patients medical record of a decision not to do CPR must include both a Physicians Order and an entry in the Progress Notes. Use of the standardized DNR Order Form is preferred, but is not required in cases where the attending physician prefers a narrative method of documentation.
For example, someone with terminal cancer might write that they do not want to be put on a respirator (breathing machine) if they stop breathing. This action can increase their control over their death and their peace of mind, and eventually reduce their suffering.
Used by Health Care Facility Staff and Regulated Health Care Providers. Submit completed order request form (available at ) to OSSDistribution@ontario.ca.
You can write an advance directive in several ways: Use a form provided by your doctor. Write your wishes down by yourself. Call your health department or state department on aging to get a form. Call a lawyer. Use a computer software package for legal documents.
