A Form SSA 11-BK is known as a Request to be Selected as Payee. It will be used by someone who wants to become a representative payee for another payee. This form must be completed in a face-to-face interview with someone from the Social Security Administration in order to determine eligibility and suitability.
As a representative payee, you are responsible for keeping records and reporting on how you spend the benefits by completing a Representative Payee Report (Form SSA-623, SSA-6230 or SSA-6233). The appropriate form will be mailed to you about once a year.
SSA-623: Representative Payee Report (Adult Beneficiaries)
Your representative payee must use your benefits to pay for your needs. These could include payment for food, shelter, clothes, medical care and personal comfort items. Also, your representative payee must tell SSA of events that could change the amount of, or affect your right to receive, benefits.
A representative payee applicant may not serve if he/she: (a) Has been convicted of a violation under section 208, 811 or 1632 of the Social Security Act. (b) Has been convicted of an offense resulting in imprisonment for more than 1 year.
You also can file the report online at .socialsecurity.gov/payee. You must complete the report even if you are the beneficiarys legal guardian. You may choose to use the worksheet in the center of this booklet to help you keep track of the money you spend.
If the applicant wishes to appoint you as his or her official representative, we will need a written statement appointing you to represent him or her in dealings with Social Security. You may use a Form SSA-1696 (Appointment of Representative) for this purpose.
A payees main duties are to use the benefits to pay for the current and future needs of the beneficiary, and properly save any benefits not needed to meet current needs. A payee must also keep records of expenses.
