A physician release to return to work form should include the employees personal information, details of the medical condition or injury, the date of the release, any restrictions or accommodations required, and the physicians signature.
A letter of intent to return to work is a formal letter an employee sends to their employer after having been away from work for a period of time. This letter could be used to announce your return from bereavement, parental leave, sick leave, or any other type of leave.
A Return to Work Certification form is included in the FMLA packet for this purpose. Please note that an employee may not return to work without first submitting a Return to Work Certification that states that the employee is able to work, listing any restrictions on normal duties.
