The Ministry of Health and Long-Term Care has specified that when a discharged patient refuses to leave a hospital, the hospital stay becomes uninsured. The hospital will then charge the patient a per diem rate. Please be prepared to leave the hospital at your scheduled discharge time.
Many hospitals have a discharge planner. This person helps coordinate the information and care youll need after you leave. Youll need to understand your injury or illness.
Hospital discharge multidisciplinary teams describe - with input from the person and their unpaid carer, family members, advocate, or relevant community-based professionals - the needs that require support after discharge before an assessment of their long-term needs.
Some hospitals have a dedicated discharge planning manager on staff, but your point person could also be a social worker, nurse, or other hospital representative. Ideally, and especially for the complicated medical conditions, discharge planning is done with a team approach.
