The most common types of advance directives are the living will and the durable power of attorney for health care (sometimes known as the medical power of attorney).
The goals of advance care planning are four-fold. These goals reflect respect for the principles of patient autonomy (right to self-determination in light of personal interests including goals, preferences, and concerns for ones family), beneficence (promoting good) and non-maleficence (avoiding harm).

Advance directives consist of (1) a living will and (2) a medical (healthcare) power of attorney. A living will describes your wishes regarding medical care. With a medical power of attorney you can appoint a person to make healthcare decisions for you in case you are unable to speak for yourself.
There are two main elements in an advance directivea living will and a durable power of attorney for health care. There are also other documents that can supplement your advance directive.
What to Include in Your Advance Directive. The name and contact information of your healthcare agent or proxy. Answers to specific questions about your preferences for care if you become unable to speak for yourself. The forms and questions asked vary a bit from state to state.
It is known, however, that advance directives (or so called living wills) pose serious practical and ethical problems related to continuity, liability, completeness, interpretation and the relationship between rejected and demanded medical treatments, all of which have been intensively discussed (see e.g., [6]).
