General Guidelines: Versions of the Bible or individual books are not underlined, italicized, or placed in quotation marks.
In this sentence, the only published work or source for published or copyrighted work referenced is Hamlet, which is italicized. This means plays are italicized. Stephen Spielberg directed Indiana Jones and the Raiders of the Lost Ark.
Remember that it is the title of a book, and book titles are normally capitalized. An oddity in English usage is, however, that Bible and the names of the various parts of the Bible are not italicized or placed between quotation marks.
Authorized King James Version, Oxford UP, 1998. In the body of your text, general references to scriptural works like the Bible, Talmud, and Koran should not be italicized unless you refer to a specific published edition.

Translators sometimes insert words to clarify the meaning and make the English text more readable. A few translations (e.g., New American Standard Bible, or NASB) italicize the added words to indicate the addition. (The italicized words are not there for emphasis, a common misunderstanding.)
The lack of italics for the Bible and its books probably has less to do with divine inspiration than with the fact that the word bible comes from Greek biblia, meaning books. The Bible, then, is the proper name for a special set of books rather than the formal title of a publication.
