Use the linkage document (registration letter) to link to your new practice. Sign in to your Patient Access account. Select Account (located under your name). Select the My GP Practice screen. Select Change my GP Practice. Select the agreement box, then select Link to my new GP practice.
If you already know which doctors surgery you wish to register with, and they are able to take you on as a new patient, the process for changing GP is super simple. All you have to do is register with the new GP and arrange for your medical records to be transferred over from your current doctors.
Linkage codes (also called linkage keys) are 3 codes that the NHS uses to protect your GP medical record and keep it secure for you. You can use these codes to access your GP medical record online and via the Nye Health App.
Because of coronavirus (COVID-19), try to avoid going into a GP surgery to register. You can: check the GP surgery website to see if you can register online. call or email the GP surgery and ask to be registered as a patient.

Link to a practice or patient Sign in with your existing details. You will only have access to a restricted account at this point. Click on your name in the top right-hand corner, then select Account. Scroll down to the Link a GP practice or person section. Select either: Enter the details from the registration letter.
You will need to contact your GP practice and ask for access to online services. You will also need to state that you would like them to send you your 3 linkage keys. Each GP practice manages these requests a little differently. Some prefer you to complete a request form online, via their website.
Changing your GP practice First obtain a linkage document (registration letter) from your new practice. This does not need to be collected in person and can be emailed to you. Contact your practice for details. Alternatively, you could change your practice using NHS login.
