The person signing must insert his or her own signature between the forward slash marks, 1.4(d)(2)(i). The name of the person signing must be printed or typed immediately adjacent (i.e., below, above, or beside) to the S-signature, and be reasonably specific, so the identity of the signer can be readily recognized.
Joint ownership - Multiple parties may together own the entire right, title and interest of the patent property. This occurs when any of the following cases exist: (A) Multiple partial assignees of the patent property; (B) Multiple inventors who have not assigned their right, title and interest; or.

Unfortunately, under United States patent law you can not amend the patent to include the new subject matter reflecting the improvement you made to the invention. You will have to file a new patent application for the improvement to the invention instead of filing an amendment to the patent.
The original owner should record the assignment or name change with the USPTOs Assignment Recordation Branch by filing a Recordation Cover Sheet along with a copy of the actual assignment or proof of name change.
If a satisfactory title is not supplied by the applicant, the examiner may, at the time of allowance, change the title by an examiners amendment.
Ownership of a patent gives the patent owner the right to exclude others from making, using, offering for sale, selling, or importing into the United States the invention claimed in the patent.
