Discomfort with digital or speculum insertion is often due to vaginismus—painful spasm of the muscles in the outer third of the vagina. This spasm may occlude the vagina, making internal examination impossible.
Unlike a Pap test, where tissue from your entire cervix gets scraped, colposcopy allows your provider to take tissue samples by scraping cells from select areas. Small pieces of tissue will be removed and sent to a laboratory to see if cancer or precancer cells are present.
Pap test. A speculum is inserted into the vagina to widen it. Then, a brush is inserted into the vagina to collect cells from the cervix. The cells are checked under a microscope for signs of disease.
Pap Smear Procedure You'll spread your legs, and your doctor will insert a metal or plastic tool (speculum) into your vagina. They'll open it so that it widens the vaginal walls. This allows them to see your cervix. Your doctor will use a swab to take a sample of cells from your cervix.
The Pap smear shouldn't hurt, but it might be uncomfortable. The good news is, it's over quickly.
A cervical biopsy may be done when abnormalities are found during a pelvic exam. It may also be done if abnormal cells are found during a Pap test.
Does a speculum hurt? It's common to feel discomfort or pressure when your healthcare provider inserts the speculum and widens it, but it should never hurt. Your provider may push down to relax the muscle at the opening of your vagina as the speculum goes in to reduce any discomfort.
A Pap smear should not hurt but may feel uncomfortable. You could experience light bleeding afterward, but you should not feel pain or cramping. Let your healthcare provider know if you feel discomfort lasting more than a few minutes or if the bleeding lasts longer than 24 hours.
What is a vaginal speculum used for? You'll most likely encounter a speculum during a pelvic exam or Pap smear. During a pelvic exam, your provider gently inserts the duck-billed part of the speculum into your vagina and opens the speculum to widen your vaginal walls.
You may feel some pressure or mild discomfort when the speculum is put in and opened. Usually a small spatula or tiny brush is used to gently collect cells from the cervix for the Pap test. You may feel a light scratching when they take the cells, or you may feel nothing at all.
