The most common pathologic process that increases afterload is systemic hypertension. Other situations that increase afterload include aortic stenosis (including subvalvular and supravalvular), aortic regurgitation and coarctation of the aorta. The afterload can be decreased by any process that lowers blood pressure.
A biventricular pacemaker has a third wire in the left ventricle, which allows timed, coordinated signals to be sent to the right and left ventricles. If needed, a biventricular pacemaker also can “shock” or defibrillate the heart, which helps control life-threatening irregular heartbeats.
Hence, afterload is increased by conditions in which blood pressure and vascular tone are elevated (Table 1). Similarly, conditions that acutely elevate RV, such as acute valvular incompetence, stress cardiomyopathy or ischaemia are associated with increased afterload.
You may feel some pain or discomfort during the first 48 hours after having a pacemaker fitted, and you'll be given pain-relieving medication. There may also be some bruising where the pacemaker was inserted. This usually passes within a few days. Tell the staff if your symptoms are persistent or severe.
The surgeon attaches the tip of the lead to your heart and the other end of the lead to the pacemaker box. This is placed in a pocket created under the skin in your abdomen (below your chest). This usually takes 1 to 2 hours, but could take longer if you are having other heart surgery at the same time.

After a pacemaker implant You should be able to get back to normal physical activities very soon afterwards. As a precaution, it is normally recommended that you avoid strenuous activities for around 3 to 4 weeks after having your pacemaker fitted. After this, you should be able to do most activities and sports.
Thus, in patients with congestive cardiac failure, increased afterload (e.g., due to phenylephrine) can cause a precipitous fall in cardiac output. Indeed, afterload reduction is a fundamental principle of the treatment of left ventricular failure.
You'll likely be awake during the surgery to implant the pacemaker, which typically takes a few hours. A specialist will insert an IV into your forearm or hand and give you a medication called a sedative to help you relax.
