Your healthcare provider will use a small brush, swab or spatula to gently remove cells from the cervix and back of the vagina. They will place the cells in a vial of liquid or smear the cells on a glass microscope slide. If you need an HPV test, your provider will take a sample of cells for this test as well.
Method Of Exam Endocervical swab: Insert a cotton applicator stick (wire brush may also be used for endocervical specimens) into the os of the cervix. Roll the stick gently between the thumb and index finger. Remove and smear a labelled glass slide.
Smears must be fixed immediately before they dry. A 95% solution of ethyl alcohol or spray fixative is used to fix the slide. The slides should be labeled in pencil with the patients name AND date of birth and a properly completed Pap smear requisition should accompany the slide.
Ethyl alcohol is the fixative specifically recommended for cytological preparations. Fixation coarsens the cell structures and sharpens nuclear chromatin pattern and its details.
The smear(s) should be immediately fixed with spray fixative or fixed in 95% ethyl alcohol for 10 minutes. Allow the slides to completely air dry before shipping. Scrape additional areas of the vagina that appear abnormal and spread and fix as noted above.
Pap staining procedure Mayer Hematoxylin and Gills hematoxylin is preferred. In regressive method the smear is over stained with nuclear stain with a non-acidified hematoxylin. The excess stain is removed with dilute HCL acid solution. Harris hematoxylin is preferred for regressive method.
Out of which, 95% ethanol is the commonly used fixative. Hence, the conventional method for fixation of Pap smear is to fix the Pap smear immediately in 95% ethyl alcohol after preparing the smear.
During a Pap test, a tool called a speculum holds the vaginal walls apart. A sample of cells from the cervix is collected using a soft brush and a flat scraping device called a spatula (1 and 2). The cells are placed in a bottle that contains a solution to preserve them (3).
