An MRI can show the extent of an ACL injury and signs of damage to other tissues in the knee, including the cartilage. Ultrasound. Using sound waves to visualize internal structures, ultrasound may be used to check for injuries in the ligaments, tendons and muscles of the knee.
In general, there are three main graft options: a patellar tendon autograft, a hamstring autograft, and an allograft. An autograft comes from the patients own body, whereas an allograft comes from a cadaver donor. An allograft can come from patellar tendon, hamstring tendon, achilles and other ankle tendons.
The ACL arises from the anteromedial aspect of the intercondylar area on the tibial plateau and passes upwards and backwards to attach to the posteromedial aspect of the lateral femoral condyle. The ACL tibial footprint substantially overlaps the anterior root lateral meniscus footprint 6.
The normal ACL should have a taut, low to intermediate signal intensity with continuous fibres in all planes and sequences. It courses parallel or steeper than the intercondylar line. The PL bundle usually has higher signal intensity than the AM bundle.
Anatomic ACL reconstruction aims to restore the function of the native ACL. Femoral and tibial tunnels should be placed in their anatomical location accounting for both the native ACL insertion site and bony landmarks.
Identify appropriate landmarksthe patellar tendon, patella, lateral femoral condyle, and lateral tibial plateauand mark them. Define the triangle between the lateral border of the patellar tendon, lateral femoral condyle, and lateral tibial plateau and identify the soft spot between these 3 structures.
The anterior cruciate ligament originates at the medial wall of the lateral femoral condyle and inserts into the middle of the intercondylar area. It contributes docHubly to the stabilization and kinematics of the knee joint.
Specific bony landmarks assessed for the ACL tibial attachment include the lateral and medial tibial eminences, the medial and lateral tibial pla- teau articular cartilage borders, the ACL ridge, the ACL tubercle, the anterolateral fossa, and the ret- roeminence ridge [1].
