hi stalks antennae and thereamp;#39;s been quite the controversy lately over whether an ACL needs to be injected with stem cells using ultrasound or whether you need fluoroscopy and so the title of my talk today is so you think a torn ACL can be injected with stem cells using ultrasound save that for the boys at the lodge so we invented and have the worldamp;#39;s largest experience with injecting ACL tears with stem cells to help patients avoid invasive ACL surgery and thereamp;#39;s a very nice published paper that seemed to show that you could inject an ACL with ultrasound and call it good these were latex injections in the ten cadaver knees with ultrasound this seemed to work to get the latex into the origin insertion however cadaver parts donamp;#39;t come with known ACL tears and as weamp;#39;ve seen time and time again injecting normal ACLs from the bottom at the tibial insertion is pretty easy you can see the stuff go all the way up like these knees but once you get into A

The EndoButton (Smith Nephew) is in an anterior position within the suprapatellar pouch.
Direct ACL insertion is located anteriorly, on the medial surface of the lateral femoral condyle. It is connected to a bone depression parallel and immediately posterior to the intercondylar lateral ridge and to the medial surface of the lateral femoral condyle.
The tibial tunnel is placed 9 mm posterior to the intermeniscal ligament or 7 to 10 mm anterior to the posterior cru- ciate ligament insertion with the center of the tunnel ideally being just posterior and medial to the anterior horn of the lateral meniscus.
The normal ACL should have a taut, low to intermediate signal intensity with continuous fibres in all planes and sequences. It courses parallel or steeper than the intercondylar line. The PL bundle usually has higher signal intensity than the AM bundle.
The contraindications are as follows: (1) Endobutton is located in the posterior aspect of the lateral femoral condyle close to the peroneal nerve, and (2) Endobutton is located far from the proximal attachment of the lateral capsule to the femoral cortex.
A radiofrequency probe is used to expose the Endobutton. If there are more than 2 buttons for double-bundle ACL reconstruction, both Endobuttons are endoscopically visualized before commencement of hardware extraction. Metzenbaum scissors are used to cut the CL, and then a rongeur is used to remove the buttons.
The anterior cruciate ligament originates at the medial wall of the lateral femoral condyle and inserts into the middle of the intercondylar area.
The Endo Button is extensively used in orthopedic surgery, particularly in the repair and rebuilding of the knees anterior cruciate ligament (ACL). It is a non-traditional method of connecting grafts to bone during ACL restoration treatments.
