If you are changing jobs, you must submit a STEM OPT Reporting Form and I-983 for both the previous job and the new job.
Due at the 12-month and 24-month mark of STEM OPT period 1. Complete and submit a signed self-evaluation (Page 5 of Form I-983) along with STEM OPT Validation Report Form.

You can track your case status at .uscis.gov. Request a tracking number when you mail your OPT application to USCIS. Track the package delivery and save the delivery confirmation with a copy of your OPT application packet.

Complete the evaluation on page 5 of your most recent Form I-983 and email the whole document to your STEM Advisor. A 6-Month STEM Validation is also due with each annual evaluation; see above instructions.
