On chest examination, the classical finding of pulmonary rales translates to heart failure of moderate-to-severe intensity. Wheezing may be present in acute decompensated heart failure. As the severity of pulmonary congestion increases, frothy and blood-tinged sputum may be seen.
Diagnostic tests for congestive heart failure may include: Resting or exercise electrocardiogram (also known as EKG, ECG, or stress test) Echocardiogram. Computed tomography (CT) scan. Magnetic resonance imaging (MRI) scan. Positron Emission Tomography (PET) scan. Biopsy or catheterization of the heart and arteries.
Once heart failure is diagnosed, treatable causes should be sought. Three classes of medicationbeta-blockers, vasodilators (especially ACE inhibitors), and spironolactonehave shown improved survival in patients with heart failure caused by reduced LVEF.
Decreased Cardiac Output related to impaired myocardial function as evidenced by signs of fatigue, dyspnea, and abnormal heart rate or blood pressure.
Evidence-based practice (EBP) is a problem-solving approach utilizing the best available information to support clinical decisions. The cardiovascular literature sufficiently supports the adoption of EBP to reduce practice variations and improve patient outcomes.
Diuretics. Diuretics (water pills) make you pass more urine and help relieve ankle swelling and breathlessness caused by heart failure. There are many different types of diuretic, but the most widely used for heart failure are furosemide (also called frusemide) and bumetanide.
Thus, in clinical practice, patients without contraindications appear to gain most benefit from combined treatment with the fantastic four: an ARNI, a beta-blocker, an MRA, and an SGLT2 inhibitor (Figure 1).
