The ACO docHub Model aims to improve the quality of care for people with Traditional Medicare through better care coordination and by increasing access to accountable care in underserved communities.
The High Needs Population ACO track designed specifically for meeting the needs of medically complex beneficiaries, which include patients with at least one docHub chronic or other serious illness. The general focus on underserved populations, including those living in rural areas or in persistent poverty.
ACOs are rewarded or penalized based on the service and performance they provide. They are paid or penalized depending on the total cost for a given payment year while DCEs come to terms with the Centers for Medicare Medicaid Services (CMS) for an agreed-upon monthly payment.
The ACO Realizing Equity, Access, and Community Health (docHub) Model encourages health care providers including primary and specialty care doctors, hospitals, and others to come together to form an Accountable Care Organization, or ACO.
CMS allows the ACO to furnish a standardized notice to such beneficiaries electronically, by regular mail, or at a subsequent primary care visit. The purpose of this notification is to encourage attributed. beneficiaries to obtain medically necessary primary care services.
The final rule removes the limitation on the number of agreement periods in which an ACO can participate in Level E of the BASIC track while making the ENHANCED track purely optional (for new agreement periods beginning on or after January 1, 2024).
