sure right so this is a case of our 23 year old footballer who met with a injury while playing football and he presented to us with instability but unlike the common injuries of football which is an antidot ligament tear this is a posterior treatment here because he fell on his knee so thatamp;#39;s how these injuries happen and one can appreciate there is a obvious sag so once I reduce the sack this is the position that I want after the surgery so there is an obvious sag and there is a soft end point upon posterior drawings test and we are trying to correct this particular abnormality in todayamp;#39;s surgery thank you thank you very much

While the anteromedial bundle is the primary restraint against anterior tibial translation, the posterolateral bundle tends to stabilize the knee near full extension, particularly against rotatory loads.
The anteromedial bundle is responsible for the posterior translation of the femur at 30 degrees flexion, and the posterolateral bundle resists hyperextension and prevents posterior translation of the femur in extension 1, 2.
The ACL arises from the anteromedial aspect of the intercondylar area on the tibial plateau and passes upwards and backwards to attach to the posteromedial aspect of the lateral femoral condyle. The ACL tibial footprint substantially overlaps the anterior root lateral meniscus footprint 6.
The anterior cruciate ligament (ACL) consists of two major fiber bundles, namely the anteromedial and posterolateral bundle. When the knee is extended, the posterolateral bundle (PL) is tight and the anteromedial (AM) bundle is moderately lax.
The anterior cruciate ligament (ACL) is an intracapsular ligament that connects anterior aspect of intercondylar eminence of tibia with medial surface of lateral femoral condyle.
The ACL was bluntly separated into the two bundles. Note that the AM bundle attaches on the cylindrical surface of the femoral intercondylar notch around its proximal/posterior outlet. PCL: Posterior cruciate ligament, AMB: Anteromedial bundle, PLB: Posterolateral bundle.
However, functionally, ACL is divided into AM and PL bundles. While the AM bundle is tense in flexion and primarily prevents anterior-posterior slippage, the PL bundle is tense in extension and is a structure that prevents the knees rotational shift. These bundles work synergistically during the knee movement.
The ACL is most commonly composed of two bands: the anteromedial and posterolateral. The anteromedial band is the stronger of the two and is taut in flexion while the posterolateral band is lax. In extension, the posterolateral band becomes taut and the anteromedial band is lax (4a).
