You can create fillable forms in Google Docs for certain use cases like gathering feedback, conducting surveys, or creating simple registration forms that require basic data collection.
In your document, with your mouse select the text to which you want to apply the style. For a main heading, click Heading 1. For a subheading click Heading 2, and so on. You do not need to apply a style to text that is not a heading (i.e. a paragraph).
To create a heading, chapter, or sub-chapter, type the text that will serve as the title, select the text with your cursor, click the Normal text drop-down in the formatting toolbar, and select Heading 1, Heading 2, or Heading 3. Continue doing this until you have all of the headings formatted.
To insert a text box: Click Insert, then select Drawing from the drop-down menu. The Drawing dialog box will appear. Click the Text box command. Click and drag in the drawing area to create the text box. Release the mouse, and a text box will appear. When you are satisfied, click Save Close.
Choose a template or start with a blank document. Create a Table for Form Fields. Create a table in your Google Docs document to hold your form fields. Add Form Controls to the Table.
