Isthmus The isthmus may be between 1.0 mm - 2.0 mm in width but is 1/3 the intercuspal width.
The inlay/onlay requires a minimum of 1.5 mm occlusal space and all cuspidian angles must be rounded and the margins must have a shoulder shape with a rounded internal angle or a wide chamfer. The preparations should have a minimum ceramic thickness of approximately 1.5 mm to 2.5 mm.
Onlays and inlays Occlusal isthmus width should be more than 1.5 mm wide, and occlusal reduction needs to be at least 1.5 mm deep.
Moderate to Large Cavities: Inlays are an excellent choice when a tooth has a moderate to large cavity that cannot be effectively treated with a standard dental filling. In such cases, an inlay provides better strength and longevity.
The outline of the isthmus width should not be wide! If it extends beyond an imaginary line that divides the cusp tip and the central groove, it is very wide already. Doing this will surely undermine the cusp and lead to fracture. To be exact about the measurement, it should be limited to 1mm in width.
A systemic review found that the most common cause of onlay failure is ceramic fracture, followed by ceramic de-bonding from the tooth structure, and the occurrence of secondary caries which is seen as a discolouration at the margins of the restoration.
The isthmus should be one-third of the intercuspal width, and the buccolingual walls should converge slightly in an occlusal direction. The mesial and distal walls should flare at the marginal ridge so as not to undercut ridges.
