The cause of revision surgery is mainly graft failure following ACL reconstruction surgery. The graft failure occurring in the first 12 months following surgery is generally a result of non-traumatic causes and the failure after 12 months is mostly due to re-injury.
The signs of ACL graft failure can include swelling, pain within the knee, locking within the knee, a mechanical block (which can be due to a bucket-handle tear of the meniscus), lack of full motion, and difficulty with twisting, turning, and pivoting.
In general, the rate of failure for a patellar tendon autograft (from ones own tissues) is lower than a hamstrings autograft. In addition, failure rates for allograft (from cadaver tissue) are higher than those for using ones own tissue.
If untreated, graft impingement can progress to graft rupture. Treatment consists of notchplasty, which is arthroscopic resection of a portion of the lateral aspect of the intercondylar notch.
While there are no definitive warning signs for ACL surgery graft failure, patients should be vigilant for symptoms such as recurrent instability, persistent pain, or swelling after surgery, as these may indicate a potential problem with the graft.
The management of anterior cruciate ligament graft impingement depends on the timing and symptom severity. Early management might involve physical and manual therapy or an extension cast. Chronic cases might require surgical revision with notchplasty and debridement 2,3,5.
If a repaired ACL fails, it can only be revised with an ACL reconstruction. Having to redo any kind of ACL surgery results in higher rates of failure, lower rates of successful return to sports activity, and increased risk of developing osteoarthritis in the knee.
