The anteromedial bundle is responsible for the posterior translation of the femur at 30 degrees flexion, and the posterolateral bundle resists hyperextension and prevents posterior translation of the femur in extension 1, 2.
It is a very useful test to determine if the patient will have a symptomatic ACL tear or not. With this test, the examiner applies an internal rotation and valgus force to the extended knee and if there is an ACL tear, the tibia will usually sublux anterolaterally on the femur.
A partial ACL tear is also called a grade 2 sprain. Grade 2 injuries are rare and occur when the ligament stretches enough to become loose and damaged. A noncontact pivoting or cutting injury, whether a pop is felt or heard, is a common cause of ACL tears. This rotational injury of the knee can lead to instability and inhibit the ability to be active in sports (see ACL injuries). Twisting injuries of the knee can also be associated with meniscus cartilage tears.
A pivot shift can be used to assess the combined tibio-femoral internal rotation and anterior tibial translation when the ACL is deficient. There are two main components of the pivot shift test: translation anterior subluxation of the lateral tibial plateau. rotation rotation of the tibia relative to the femur.
The classic bone marrow edema pattern seen following the pivot shift injury involves the posterolateral tibial plateau and the midportion of the lateral femoral condyle. Edema occurs in the anterior aspect of the prox- imal tibia following the dashboard injury.
