Put commas between the name and credentials, as well as between credentials (Kelly Smith, MD, PhD). Credentials should be listed in the following order: Highest academic degree earned, licensure, certification (John Smith, DNS, RN, FNP-BC). Do not put Dr. before a name unless quoting someone.
Rules for titles: If a title follows a name, separate the title from the rest of the sentence with a pair of commas. Example: Sandra Belinsky, MD, has been appointed to the board.
In an address, place a comma after the street and between the city and state: 1714 North Harvey Street, Griffith, IN 46300. Do not place a comma between the state and the ZIP code.
Levin, MD, section chief, Oncology. Formal titles that appear before a persons name are in uppercase: Oncology Section Chief Stanley A. Levin, MD. Place a comma after the last credential after a persons name if including their title, department or other information: Stanley A. Levin, MD, section chief, Oncology.
When writing an address all on one line or in a sentence, use a comma before the following elements: the apartment or suite number, the city, and the state. Its not necessary to use a comma before the zip code. Her address is 3425 Stone Street, Apt. 2A, Jacksonville, FL 39404.
Fortunately, it is easy to learn this. If someone or something is being addressed in a sentence, then a comma or pair of commas is necessary. But if someone or something is the object of the sentencethe thing to which an action is donethen no commas are required.
The correct way to punctuate MD (Medical Doctor) is M.D. Note: In English, there are two main punctuation styles: British and American. They mostly differ in how they treat quotation marks, particularly when combined with other punctuation marks.
In academic degree abbreviations, do not use periods. The exact degree is uppercase, but the subject is lowercased in masters degrees.
