welcome back to acl academy this is dr khan today we are going to touch on acl graph choices so what over youamp;#39;re going to use for your new acl uh before i get into that if you guys have found value in the page please subscribe to the channel like this video and share with a friend spread the word so when weamp;#39;re talking about acl um reconstruction uh we are talking about replacing the native acl ligament so when itamp;#39;s torn that area does not have great blood supply usually a repair historically a repair is not possible and it needs to be replaced there are some new technologies coming out that weamp;#39;re starting to utilize for repair procedures having said that the evidence is just not there yet uh so currently weamp;#39;re still seeing a lot more reconstructions which is replacing the native uh acl versus repairs which is basically taking the tear and repairing it okay so when weamp;#39;re talking about reconstruction well we have to use something to replace

It is a non-traditional method of connecting grafts to bone during ACL restoration treatments. The Endo Button is a fixation device that secures and reliably attaches the graft to the bone. It comprises of a small metallic button, usually made of titanium, that is affixed to the grafts looped or folded end.
The tibial tunnel is placed 9 mm posterior to the intermeniscal ligament or 7 to 10 mm anterior to the posterior cru- ciate ligament insertion with the center of the tunnel ideally being just posterior and medial to the anterior horn of the lateral meniscus.
The patellar tendon and hamstring autografts are the most common choices for ACL reconstruction. The patellar tendon runs from the knee cap (patella) to the lower leg bone (tibia). Surgeons have the most experience with this autograft and it is the most widely used.
ACL surgery involves reconstructing the ligament. Surgeons use screws to attach the reconstructed ligament to the femur (thigh bone) and the tibia (shin bone).
The ACL tunnel was created to not exceed an angle of 65 with respect to the tibial plateau in the sagittal plane.
Ideally, the tunnel placement should be centered one-quarter of the total facet length anterior to the posterior cortex of the tibia (7).
Specific bony landmarks assessed for the ACL tibial attachment include the lateral and medial tibial eminences, the medial and lateral tibial pla- teau articular cartilage borders, the ACL ridge, the ACL tubercle, the anterolateral fossa, and the ret- roeminence ridge [1].
The femoral tunnel should normally be posterior to the junction of the posterior cortical line of femur and roof of intercondylar notch. In this case, it is seen anterior to this point.
There is earlier incorporation into the body with an autograft because the tendon is from your own body. Additionally, there is no potential for outside disease transmission or rejection of the tendon. An autograft is also stronger than an allograft because of the sterilization process for donor tissue.
