For infants and younger children receiving more than two injections in a single limb, the thigh is the preferred site because of the greater muscle mass. For older children and adults, the deltoid muscle can be used for more than one intramuscular injection.
Here is a simple technique that can be used from premolar to premolar to give an pain free palatal injection.
Needle insertion Insert needle at an 45o angle to the skin. Pinch up on SQ tissue to prevent injecting into muscle. Aspiration before injection is not required. Multiple injections given in the same extremity should be separated as far as possible (preferably at least 1 apart).
Injection site Deltoid: Give in the central and thickest portion of the deltoid muscle above the level of the armpit and approximately 23 fingerbreadths (~2) below the acromion process (see diagram 1). To avoid causing an injury, do not inject too high (near the acromion process) or too low.
Avoid injecting in the exact same spot. Using the same spot every time can form bumps or pits in the skin. For example, inject your insulin in your left upper arm, then the next time in your upper thigh, then the next time in your left upper arm again but slightly lower than before.
An intramuscular injection delivers medication into a muscle. Doctors frequently use intramuscular injections to administer vaccines and certain other drugs. Injection sites include the upper arm, hip, thigh, and buttocks.
Intradermal injections (ID) are administered into the dermis just below the epidermis. Use a tuberculin syringe, calibrated in tenths and hundredths of a milliliter, with a needle length of 1/4 inches to 1/2 inches and a gauge of 25 27 Page 2 ID injections may be given on the inner aspect of the forearm.
Intramuscular injections are often given in the following areas: Deltoid muscle of the arm. The deltoid muscle is the site most typically used for vaccines. Vastus lateralis muscle of the thigh. Ventrogluteal muscle of the hip. Dorsogluteal muscles of the buttocks.
