Lumbar facet joint injections. Facet joints arelittle joints situated throughout the spinal column. They can be a common source of back pain. A facet joint injection can be helpful in diagnosing and alleviating back pain associated with facet joints. The procedure is done under local anesthetic. The doctor will initially numb the skin and deeper tissues with the numbing medicine. A special x-ray machine, called a fluoroscope, is then used to guide the needle accurately into the facet joints. A mixture of local anesthetic and steroid is injected slowly into the joints. The local anesthetic can provide immediate pain relief but the steroid can take a few days to work. Pain relief from facet joint injections can last several months though this may not always be the case.

An injection of the joint is still reported with 27096. Injections of the nerves innervating the SI joint would be reported with 64451.
Outpatient hospitals report G0260 for sacroiliac joint (SI) joint injections. Hospital outpatient coders do not code for the image guidance. For physician coding, CPT code 27096 is reported for SI joint injection. This code does include image guidance.
