When patients and family members are asked about their preferences for end-of-life care, they often face decisions about signing specific documents, including a Do Not Intubate order (DNI) or a Do Not Resuscitate order (DNR).
A DNR order is allowed for any reason. Any adult can request a DNR, including healthy adults. People most likely to use them are those who know in advance that their chance of surviving CPR is very low and/or they dont want to spend their last days on life support.
DNR means that no CPR (chest compressions, cardiac drugs, or placement of a breathing tube) will be performed. A DNI or Do Not Intubate order means that chest compressions and cardiac drugs may be used, but no breathing tube will be placed.
Strictly defined, a do not resuscitate/do not intubate (DNR/DNI) status indicates a patient does not want to receive CPR or intubation in the event of cardiopulmonary arrest.
In medical terms, a DNI means that patients do not want a breathing tube inserted into their trachea, via the nose or mouth, to restore normal breathing. If the patients breathing remains impaired, the tube remains in place and is eventually connected to a ventilator to maintain breathing.
Generally, a DNR order should not be entered into the patients medical record without concurrence of the parents. However, a DNR order may be entered by the patients physician without parental permission if CPR is determined to be physiologically futile or non-therapeutic, in ance with Paragraphs E and F below.
Can you have DNI orders without DNR orders, and vice versa? Most often a DNR order also includes a DNI order. This is because restarting someones heart requires supporting their breathing, too. But DNI orders can be separate from DNR orders.
