Carotid cavernous fistula (CCF) is an abnormal shunt from the carotid artery to the cavernous sinus. The symptomatology of CCFs is mainly a result of the effects on important neural and vascular structures in the cavernous sinus which include cranial nerves III, IV, V1, V2, and VI.
Direct CCF often occurs days or weeks after a closed head injury. Patients present with the classic triad of chemosis (red eye syndrome), pulsatile exophthalmos (abnormal protrusion of the eyeball), and ocular bruit (blood flow sounds coming from the eye).
Direct CCFs are rarely asymptomatic and are treated urgently. For symptomatic CCFs, endovascular treatment is first line therapy with the approach to obliterate the fistulous connection (i.e. transarterial, transvenous or transorbital) dependent on arterial supply, venous drainage and pathophysiology.
Risks of a Carotid-Cavernous Fistula Most people with indirect fistulas are better within 6 months of treatment. Sometimes people who have a direct fistula have lasting side effects, including: Bulging eye. Vision loss.
