there are between five and ten percent of medical claim denials on average ing to the aafp but this isnt necessarily a set average there are many different studies that yield different results though not many of these results seem to fall below that range another study by the centers of Medicare and Medicaid services or CMS for instance report differently in 2020 across healthcare.gov insurers reports show that 18 of in-network claims resulted in claim denials these denial rates vary from Individual providers as well this is largely due to the lack of standardization within the Health Care system in general and the good news is there is a standardization when it comes to the use of denial codes in medical billing hi everyone Im Matt from me tactics and today Im going to talk to you about the Nile codes in medical billing now before we get started though make sure you subscribe to our YouTube channel by clicking the button below while youre down there hit that alertable icon next to

The IM or SQ injection can be billed more than once or twice. If the drug is prepared and drawn up into two separate syringes and it is then administered in two individual injections in two distinct anatomic sites, you can bill two units of code 96372 (billing second unit with modifier 76).
CPT code 96372: Injection of drug or substance under skin or into muscle.
The 96372 CPT code is to be billed for each injection performed on a patient. Modifier 59 should be used when the injection is a separate service from other treatments.
It is true that an evaluation and management code, an E/M or office visit, can be reported with a minor procedure such as an injection, but only if the E/M is docHub and separate and exceeds the pre-service evaluation that is inherent to the injection.
Answer: CPT code 96372 should be reported for each intramuscular (IM) injection performed. Therefore, if two or three injections are performed, it would be appropriate to separately report code 96372 for each injection.
As weve already established, if a patient is given more than one subcutaneous or intramuscular injection, then each injection should be invoiced with a separate CPT code. Modifier 59 will then be used along with the second and every other subsequent injection code billed on the claim form.
Intraarterial Injection Additional sequential IV push is reported with add-on codes 96375 for a new drug or substance or 96376 for the same drug or substance when provide in a facility. Once again, all drugs are to be separately using HCPCS national level II codes or CPT code 99070.
Answer: CPT code 96372 should be reported for each intramuscular (IM) injection performed. Therefore, if two or three injections are performed, it would be appropriate to separately report code 96372 for each injection.
When the injection/infusion code is billed with an Evaluation Management (E/M) visit, a modifier code must be appended to the E/M code to ensure that both services are paid when appropriate. Modifier 25 would generally be used for this purpose, if criteria for the use of this modifier are met.
Our practice use cpt 96372 and an in-house code with description, Medication Supplied by Patient.
