An issue log is a simple list or spreadsheet that helps managers track the issues that arise in a project and prioritize a response to them.
Risks tend to be thought of as having an adverse impact on the project, but there are also positive risks. Actions are what you need to do throughout the project. Issues are when something goes wrong in the project.
The issue log is where you record any problems that were not accounted for in the plan and that threaten to delay the project, push it off budget or reduce the scope (e.g. reduce product performance). Issue Log. Risk Register. Description of the issue. Description of the risk.
What should an issue log include? The issue type and name. A technical description of the issue. The name of the person who reported the issue. The name of the person responsible for solving the problem. The date or time of the issue. The issues priority and status. The date or time that the problem was resolved.
An action log is an XML document that contains a set of transactions. Each transaction contains one or more actions. Each action has a name and input parameters. Some actions also have output parameters. For more information, see Transactions and Actions.
The actions log records information such as owner, due date, completion date, et cetera for things that need to be done. The issues log records information such as issue description, owner, resolution, et cetera for known problems that have occurred (if a risk triggers, it becomes an issue as well).

The issue log, sometimes also known as an issue register, is a project document where all issues that are negatively affecting the project are recorded and tracked. When an issue log is created, it provides a tool for reporting and communicating all that is happening within the project.
Shrivastava: I call the issue register the issue log because it denotes logging of something that already has happened. In the risk register, I include the probability, impact, response strategy and also the timeframe for the risk event.
