Welcome back to my channel, Adventures in America! Today's topic is how to fill out the CDC Attestation Form. This form is required for all passengers traveling to the US. The latest version expires on 6/30/2022. Parents of children under two no longer need to complete the form. Section one is for US citizens, and section two is for non-US citizens. The form can be found on the CDC website. Let me show you how to fill it out!
