Make sure your donation form includes these essential elements: Name. This is important for your own data collection. Contact Information. For example, when recording an in-kind donation for donated office space with a fair value of $40,000, you should record the following: Debit rent expense $40,000. Credit in-kind contribution rent $40,000.
