If workers are in an environment which stresses safety but does not include near-miss reporting as part of its safety plan, they may feel that they are derailing safety progress by reporting their near miss. Reputation. Workers do not want to be seen as overly reactive or accident-prone.
It should include: the names and positions of the people involved. the names of any witnesses. the exact location and/or address of the incident. the exact time and date of the occurrence. a detailed and clear description of what exactly happened. a description of the injuries.
Recognizing employees and providing incentive programs is a simple yet effective way to encourage near miss reporting. An effective incentive program will focus on rewarding employees for reporting hazards and taking an active role in creating a safer workplace.
Make reporting anonymous. Employees should be able to report concerns about hazards and unsafe conditions without fear of retaliation. If youre requiring people to sign their name on the near miss report, they may fear you will use the information against them and youll see near miss reporting drop.
An incident report should be objective and supported by facts. Avoid including emotional, opinionated, and biased statements in the incident report. It should provide both sides of the story and should not favor one side.
