The most important things to remember are: If the main subject of the image is a person even if the persons face is not visible it will require a model release. Images of people photographed from behind or without their permission or awareness will generally not be acceptable.
You will need the models signature, documenting that they agree with the statements in the release form. You will need the contact information for the model. You will also need a signature of a third-party witness, who can confirm that it is signed by the model in the picture.
Generally, you do not need a model release for photographs you publish on your website portfolio or use in your photography portfolio. There are some exceptions, however.
