A medical directive is something Ive discussed with a variety of people over the years in my role as a director of the Hospice program and as a medical oncologist here at McFarland Clinic. Its a difficult discussion, but a very important one to have with you and your family. Lets start with the basics. What is a medical directive? A medical directive is what you do each and every time you visit with your physician, or any healthcare provider. You tell them whats important to you and how you would like your care delivered. Of course, you listen to what they have to say. You factor that into whats important to you, and then, hopefully, you share those decisions with your family members. The most common types of advance directives are the living will and the durable power of attorney for health care (sometimes known as the medical power of attorney).
The date of a contract is established one of two waysby stating the date in the introductory clause or by having the parties date their signatures.
Should the documents have the same date, the advance directive prevails.
The Advance Care Planning - Goals of Care are categorized as Comfort Care (C), Medical Care (M), and Resuscitation (R).

They may also serve to formally assign a substitute decision-maker in the event the patient becomes incapacitated.
An Advance Directive does not name a person to act on your behalf. A Representation Agreement is the way in BC to legally choose and authorize a person to help you with health care decisions.
