Trapped moisture, when ramped up to solder temperatures creates steam per se, pressure and out-gasses. Solder gets expelled from the solder joint, displaced, creating a ball. Keep in mind when using aqueous cleaning of PCBAs that they should be baked out prior to hand assembly or thru-hole.

A tip in bad condition The photo shows that the tip end is oxidized and repelling solder. This means the tip is in bad condition for heat transfer.
Begin by cleaning the surface with a metal cleaner or degreaser. After cleaning and once the area has dried, spray the Original WD-40 Formula across the surface and let it sit for several minutes. Scrub the rusted area with a brush or steel wool until the rust is removed.
If your soldering iron tip is either dirty or damaged, it will stop the iron from transferring heat and therefore the solder wont melt. If the tip is damaged, then it will need to be replaced. If the soldering iron tip is still generating some heat, than the tip should be tinned or cleaned.
This issue is usually caused by the soldering iron being set to an improper temperature, or the iron itself has a wattage that is too low for the solder size you are using. If you notice your solder isnt sticking, check your solder material and compare it to the wattage of your soldering iron.
Always tin your tips This protective layer prevents your soldering iron tip from oxidising. It also makes soldering easier, as the layer creates a heat connection between your tip and the part you are soldering, which increases the efficiency of the heat transfer. Before you tin your tip, make sure it is clean and dry.
To tin your tips, first heat up the soldering iron to the proper operating temperature for your particular solder or project. Then wipe the tip on a slightly damp sponge or dry brass wool to clean it. Next you want to apply a small bead of solder to the tip (the copper part).
