An adoption petition contains the phrase, Will be known as. Changing the last name can be done at adoption, but it doesnt have to be. An older child could elect to keep their last name.

Can I get a copy of my original birth certificate? We cannot issue a copy of the original (sealed) birth certificate without a court order from the Superior Court of the county of residence of the adopted child, or the Superior Court of the county granting the order of adoption.
What happens to an adopted persons original birth certificate after a new amended birth certificate is issued? The original birth certificate is typically sealed and made unavailable to the public and, in many states, unavailable to the adopted person, even as an adult.
Vital Statistics Unit. Get a plain paper copy of your original birth certificate from the Vital Statistics Unit if you are an adult adoptee and you can identify the names of the parents listed on your original birth certificate.
In Florida, adoption records are sealed. You need a court order to obtain copies of the original birth certificate. See a list of the Florida Clerks of Court.
If you want to obtain your amended birth certificate after adoption, you can request it simply by completing the online form, paying the government fees and submitting it to your birth states Vital Records office. However, adult adoptees do not have automatic access to their sealed original birth certificates.
