Harrington rods can bend, break loose from the wires, or worse, break completely in two, necessitating further surgical intervention and removal of the rod. The ideal candidate for vertebral body tethering will be an individual who is older than 10 years of age but who has not yet reached spinal maturity. The degree of spinal curvature should be advanced and fall within a range of 35-70 degrees of curvature.
For the first time in 40 years, we do have some truly new approaches to treating severe scoliosis. One form of non-fusion scoliosis surgery is posterior dynamic distraction. So instead of placing two metal rods and screws in the back, the surgical procedure is three screws and one rod.
The potential disadvantages include increased operating time over Harrington rod alone, possible neural injury, and failure of instrumentation.
The Harrington rod was a stainless steel spinal instrument that was implanted in the patient along their spinal column to treat a curvature in their spine starting in the 1960s. Harrington rods were the most common tool used for scoliosis to treat instability and deformity of the spine from 1967 until about 1990.
Patients treated with Harrington rods often do well for years or even decades. The spine can compensate for the "flattening" of lordosis with normal discs below the fusion. Eventually, as the discs below the fusion wear out (degenerate), the patient loses the ability to stand upright and develops pain.
