I was a caregiver for my brother basically for his entire adult life even before he got sick. My brother could not read and write, so I was his caregiver and I went along with him to his hospital visits, his doctor visits, just to explain to him exactly what was going on and make sure he understood the things he was signing and that sort of thing. He had some liver issues, he was diabetic. And unfortunately we werent aware of Advance Directives. (soft music) Advance Directive is a legal document that shares with everyone what you will like people to know about your wishes. I dont think Ive realized the task at hand, how important it was until he actually did get sick. And unfortunately we didnt have that discussion about what he wanted if he was incapacitated and unable to make decisions for himself. So it came down to he had gotten sick, and we had to have these very difficult discussions after he was will. Its really important because a lot of times when youre sick,

The law clearly bars doctors from giving unwanted treatments. Even when the written directive is unclear, if a physician ignores an agents attempts to enforce the patients preferences, there are grounds for a lawsuit.
Substantial changes, such as changing your Agents or medical treatment wishes, require a new form. It is important that your form is easy to understand and that there be no questions regarding your wishes. If you do make changes, be sure to destroy old copies and share new copies with your doctor and agent.
A patient can revoke an advance directive at any time hes capable of making decisions. For example, he may change his mind about his previous decision if his condition changes for the better or worse. He can revoke an advance directive orally or, preferably, in writing at any time.
Yes, at any time, you can cancel or change any advance health care directive that you have written. To cancel your directive, simply destroy the original document and tell your family, friends, doctor, and anyone else with a copy that it is no longer valid.
States what type of medical treatment is desired. Can be general or very specific. Designates another person to make healthcare decisions. The patient is responsible for making sure an advance directive is followed.
In short, it is possible for a doctor or your appointed healthcare agent to make medical decisions on your behalf that are different from what you write in your advance directive. Any other person would not have the opportunity to override your advance directive.
A basic problem with creating a very specific living will is that hardly anyone can anticipate, years ahead of time, what treatments and interventions they will want or not want in circumstances they have never faced and have little experience of.
The goals of advance care planning are four-fold. These goals reflect respect for the principles of patient autonomy (right to self-determination in light of personal interests including goals, preferences, and concerns for ones family), beneficence (promoting good) and non-maleficence (avoiding harm).
