SSA uses the information on Form SSA-4547 to select a qualified representative payee in order of priority.
We recommend that you title the account in one of the following ways: (Beneficiarys name) by (your name), representative payee. (Your name), representative payee for (beneficiarys name). You can also file the report online at .socialsecurity.gov/payee. You must complete the report even if youre the beneficiarys legal guardian. You may choose to use the worksheet on pages 16-17 to help you keep track of the money you spend. Under the new statutory provision, representative payees who are parents or legal guardians living with their child, parents living with an adult disabled child, and spouses are no longer required to file an annual report accounting for how they spend the childs or spouses benefits.
You must complete form SSA-11 (Request to be selected as payee) and show us documents to prove your identity. You will need to provide your social security number, or if you represent an organization, the organizations employer identification number.
Generally, the payee is the person to whom you make the payment, regardless of whether that person is the beneficial owner of the income. However, there are situations in which the payee is a person other than the one to whom you actually make a payment.
This is your Representative Payee Report. You are required to file it when the beneficiary dies, when you are no longer serving as the beneficiarys representative payee, or at OWCPs request.
