When I create Excel tools, I always add an instruction sheet. But it can be a pain to create instructions, and itamp;#39;s double-pain if you write them in Excel. This is what happens to me all the time. I create a nice instruction sheet, take away the grid lines, put in light borders and start typing. All good here. A Note To File (NTF) may be written by any member of the study team to provide additional information or clarification when other documentation is unavailable or inadequate elsewhere in the research records.
File notes are generally for more minor issues or issues where you are not able to or its not appropriate to enter into formal disciplinary procedures. File notes can either sit in an employees personal folder or within another secure filing area only accessible by HR and managers.
A note to file should: Be generated on a case-by-case basis. Include the subject and protocol it refers to. Be signed and dated by the individual who is writing it. Be legible if handwritten. Explain clearly and specifically the reason for the error/omission/discrepancy or process/policy it aims to address.
File notes must include the following details: Client Name. Adviser/staff members name present at the client contact. Date of conversation. Whether this was via telephone or a face to face meeting. Subject of the conversation. Outline of the discussion. Outstanding Action (if applicable)
